The problem with rural hospitals is a state issue, not a Federal issue, because it's the state overrulations that are causing the problems. He can't do anything about the schools. Those are all local now. Like the rural hospitals, the most he can do is send or withhold Federal money. In both instances, he's using those issues to hide the fact that he doesn't care about the country and is, effectively, voting to shut down the gov't.
