Add Markdown-to-HTML web content handler (md_handler.js)

Similar to asc_handler.js, converts .md files to HTML on the fly.
Supports headings, bold/italic, code blocks, lists, blockquotes,
links, images, horizontal rules, and pipe-delimited tables.
