I see several cellphone companies offering "international" SIM cards. So, it wouldn't have to be restricted to the +7 country code that you mentioned. I could probably pose as Joe Smo in the UK, sign up for a phone plan there, and have the card mailed to me in Canada. If payment can be arranged via Bitcoin, then that would add another layer of obfuscation. But even a bitcoin payment would have a paper-trail to the source of funds. And... there will be a paper trail left by using the postal system.
