You are mixing a few things up here: * An injection like against the flu already has the antibodies, the idea is not to make you sick to create the antibodies but not to get sick .... at all. * Smallpox is good for 10 years, no need to have it every year. In this case you get an actual weakened virus, not antibodies. Right now the WHO has considered smallpox eradicated. I have had such a coktail also in the military, and yes ... you do get sick of that for a few days, but not because it is a bodily reaction against a live (weakened) virus. It just reacts against the cocktail itself. I remember feeling a little bit unwell for a short time and then we decided to go play some sports. The uneasyness was quickly gone once one became active.
