The basic task in inpatient psych: 
every 15 minutes someone on nursing staff 
has to lay eyes on every patient on the 
unit and document. These are the "obs." 

Breanna was in the dayroom, awake, alert, 
and cooperative. Paige was in her 
bathroom, awake and alert, tearful, and 
withdrawn. Mark was in the hallway, awake 
and alert, uncooperative, agitated, 
defiant, and out of control. Hot cheeto 
was in her room, sleeping, left side.

When a patient's asleep, shine a flashlight 
and count three breaths, then document the 
ob. Every 15 minutes, without fail. No 
matter what happens.
